Before contacting the rooms

Where possible, please have:

  • Your GP or specialist referral
  • Relevant scans, blood tests or pathology reports
  • Your Medicare and private health insurance details
  • Your preferred consulting location
Your GP can send the referral directly.
You do not need to upload clinical documents through this website.
For GPs and specialists

Send a referral.

Please send the referral letter and relevant investigation results directly to the rooms. Urgent referrals can be discussed by phone.

Please include

  • Patient name, date of birth and contact details
  • Reason for referral and relevant symptoms
  • Clinical urgency where applicable
  • Relevant blood tests, imaging and pathology
  • Current medications and relevant medical history
  • Referrer details, provider number, signature and date
